Concorde Health consists of four NHS GP Practices. We are all based in Hillingdon, Uxbridge, Middlesex. We have come together and formed our own Network. We hope to provide a better range of services to our patients as a Network. The Network consists of the following four GP Practices:

Services

We provide a number of services as a network. We provide an integrated care service which involves carrying out care plans on the most appropriate patients who are aged 65 and over. We also carry out health checks on our patients aged 75 and over. We hope to be taking on more services in the near future.

Concorde Health works closely with Age UK's Hillingdon Primary Care Navigator scheme. This has contributed significantly to the health and well-being of patients in the London Borough of Hillingdon by linking them to a range of community services and reducing the need for unplanned care.

The primary care navigator(PCN) enables patients aged 65 and over and their carers to access to a wide range of health, social care and voluntary sector services in the community and helps to ensure that there is a coordinated approach to that support. PCNs work across a number of allocated GP surgeries and visit patients in their own homes as well as in other community settings. Although primarily working from the surgeries, the care navigators maintain a strong working relationship with Age UK Hillingdon, working closely with staff and volunteers.

Concorde Health also works with the Hillingdon Rapid Response Service. This service provides a single point of contact for adult patients experiencing a health crisis who could be safely cared for in the community instead of being admitted to hospital.
It is one of a range of intermediate care servizces which provide nursing, therapies and rehabilitation in community settings for adults.
The multi-disciplinary team assesses and initiates services in order to support the patient, within two hours of referral. They work closely with the patient’s general practitioner (GP), district nurses and other community health, social and voluntary services to identity the most appropriate and safe package of care.
The types of conditions or situation that might require a rapid response might include:
  • Falls without apparent injuries
  • Urinary tract infection, respiratory tract infection, cellulitis
  • Exacerbations of chronic obstructive pulmonary disease (COPD) or congestive heart failure (CHF)
  • Carer has become incapacitated or there is an emergent change in carer situation (the major carer is unavailable)
  • Dehydration
  • Unstable diabetes
  • Patients at end of life
  • Elderly frail patients who have become symptomatic and are at risk of admission
